Appointment Information

You can book an appointment in several ways:

  1. Online: Login to your patient portal, click "Book Appointment," select your preferred department, physician, and available time slot.
  2. Phone: Call our appointment line at +27 11 123 4567 during business hours.
  3. WhatsApp: Send a message to +27 11 123 4567 with your name, preferred date/time, and specialist type.
  4. In Person: Visit our reception desk to schedule your next appointment.

Note: For urgent medical issues, please call our emergency line or visit the nearest emergency room.

To reschedule or cancel an appointment:

  1. Online: Login to your patient portal, go to "My Appointments," and select the appointment you wish to modify. Click "Reschedule" or "Cancel" and follow the prompts.
  2. Phone: Call us at +27 11 123 4567 at least 24 hours before your scheduled appointment.

Important: Please cancel or reschedule appointments at least 24 hours in advance to avoid a cancellation fee and to allow other patients to use the time slot.

Please bring the following items to your appointment:

  • Government-issued photo ID (ID card, passport, driver's license)
  • Medical aid card and authorization number (if applicable)
  • Referral letter (if you were referred by another doctor)
  • List of current medications (including dosages)
  • Recent test results or medical records related to your visit
  • List of questions or concerns you want to discuss
  • Payment method for co-pays or self-pay services

First-time patients: Please arrive 15-20 minutes early to complete registration forms, or complete them online through the patient portal before your visit.

Yes, Gardenview Medical Centre offers telehealth consultations for many types of visits. Virtual consultations are available for:

  • Follow-up appointments
  • Medication management
  • Minor illness assessment
  • Chronic disease management
  • Mental health consultations

To book a telehealth appointment:

  1. Login to your patient portal
  2. Select "Book Appointment"
  3. Choose "Virtual Visit" as the appointment type
  4. Select your provider and preferred time slot

Note: Not all conditions are suitable for telehealth. Our staff will help determine if a virtual visit is appropriate for your specific needs.

Patient Portal Assistance

If you've forgotten your password, you can easily reset it by following these steps:

  1. Go to the patient portal login page
  2. Click on the "Forgot Password" link below the login form
  3. Enter the email address associated with your account
  4. Check your email for a password reset link (check spam/junk folders if not received within 5 minutes)
  5. Follow the link to create a new password

Your new password must:

  • Be at least 8 characters long
  • Include at least one uppercase letter, one lowercase letter, and one number
  • Not match your previous 3 passwords

Trouble resetting? If you don't receive a reset email or encounter any issues, please contact our support team at help@gardenviewmedical.co.za or call +27 11 123 4567 during business hours.

To create a new patient portal account:

  1. Visit our patient portal
  2. Click "Create Account" or "Register"
  3. Enter your personal information, including:
    • Full name (as it appears on your ID)
    • Date of birth
    • Email address
    • Mobile number
    • ID or passport number
  4. Create a secure password
  5. Accept the terms and conditions
  6. Verify your email address by clicking the link sent to your inbox

Note: For security purposes, we will need to verify your identity before you can access certain features. You may be asked to provide additional verification during your first login or when accessing sensitive information.

Billing & Insurance Information

Gardenview Medical Centre accepts the following payment methods:

Payments can be made:

  • In person at our reception
  • Through our patient portal
  • Via EFT to our bank account

Payment Plans: If you need assistance with payment, please speak with our billing department about available payment plan options.

Medical Records Information

You can request your medical records in several ways:

  1. Online: Through the patient portal, navigate to "Medical Records" and select "Request Records"
  2. In Person: Visit our reception desk and complete a medical records release form
  3. Email: Send a request to records@gardenviewmedical.co.za
  4. Mail: Download the Medical Records Request Form, complete it, and mail it to our facility

When requesting records, please specify:

  • Your full name and date of birth
  • Contact information
  • Specific records you're requesting (or "complete medical record")
  • Date range for the requested records
  • Whether you want electronic or paper copies
  • Where to send the records (to you or to another provider)

Processing Time: Record requests are typically processed within 7-10 business days. A nominal fee may apply for printed copies or extensive records.
